CDPAP is a Medicaid-funded program that allows New Yorkers to engage their own caregivers, such as a daughter, son, family, friend, or previously hired assistance. CareConnect is the major supplier of CDPAP

STEP 1

Assessment

You will be visited by the nurse at your home to see if you qualify for the CDPAP program. You’ll learn how many weekly hours of care you’re eligible for.

STEP 2

Caregiver Choice

You select a caregiver from among your family members: your daughter, son, relative, or acquaintance. To enroll you in the program, we come to your home.

STEP 3

Peace of mind

You’re looked after by someone who genuinely cares about you. They have peace of mind because they are compensated for the services they do.

Your caregiver is dedicated to you.
They should not have to wait for payment.
